There are up to 30 residents in service at the IHC Family Medicine Center, and each year, up to 10 of them graduate from this program. This collaboration enriches our community with cutting-edge medical expertise and compassionate care.
In honor of Disability Awareness Month, we are thrilled to feature Dr. Judy Fitzpatrick on our latest IHC Podcast episode. Dr. Fitzpatrick, a recent Stanford-O'Connor Family Medicine Residency Program graduate, has a deep passion for preventive health and empowering individuals with disabilities.
In this episode, Dr. Fitzpatrick shares her story about her differently-abled older brother and how his journey inspired her to pursue a healthcare career. We are delighted to congratulate the Stanford School of Medicine residents honored during an intimate ceremony on June 15th. This event marked the culmination of their three-year residency at the Indian Health Center of Santa Clara Valley.
These dedicated doctors have demonstrated unwavering commitment and exceptional hard work over the past three years, serving patients of all ages at our Family Medicine Center. The serene setting of Saratoga Springs provided the perfect backdrop for a familial gathering of faculty, colleagues, friends, and family, all coming together to celebrate this remarkable achievement.
We extend our heartfelt gratitude to everyone who contributed to the success of these outstanding doctors in their journey. Your support and encouragement have been invaluable.
A special thank you goes to IHC Chief Medical Officer Dr. McCraven for delivering heartfelt and inspiring words of gratitude and motivation for our agency and the community we proudly serve.

Stellar Patient Navigator Staff Work!
|
|
Diabetes and hypertension often coexist, significantly increasing the risk of cardiovascular events. Hypertension is prevalent in about 75% of individuals with type 2 diabetes, and those with diabetes are twice as likely to develop high blood pressure compared to those without. If untreated, hypertension can lead to severe complications such as heart disease and stroke. The World Health Organization predicts 366 million people worldwide will have diabetes by 2030, highlighting its epidemic status. There is also an inverse socioeconomic gradient in mortality rates among patients with type 2 diabetes, doubling in lower socioeconomic strata compared to higher ones.
Recognizing these critical public health issues, IHC has developed specialized programs to help diabetic and hypertensive patients manage their conditions. Central to the success of these programs are our dedicated patient navigators, who provide exceptional support and guidance. The Hypertension program uses Remote Patient Monitoring (RPM) for continuous monitoring and early intervention, while the Diabetes Mellitus program focuses on closing care gaps through essential health maintenance exams. Both programs offer active outreach, case management support, and access to ancillary services, fostering collaboration with clinical teams.
Recent accolades from the Santa Clara Family Health Plan commend IHC for significant improvements in HEDIS rates for hypertension and diabetes management, reflecting the dedication and effectiveness of our patient navigators. Their stellar work has led to tangible clinical benefits, such as reduced blood pressure levels among consistent RPM participants and improved hypertension control. Our patient navigators ensure patients stay on track with their health goals through frequent follow-ups and re-engagement efforts. Congratulations to our outstanding team for their invaluable contributions to improving patient health outcomes.

Santa Clara Family Health Plan Visits IHC

On July 27th, we had the pleasure of hosting a visit from the Santa Clara Family Health Plan (SCFHP). Their esteemed team included Dr. Ria Paul, Chief Medical Officer, along with QI staff members Mai Chang, Anthony Lin, and Jasmin Rodriguez. During this visit, Dr. McCraven and Daniela Arcinega received the Provider Performance Program bonus check for the 2023 work year on behalf of the Indian Health Center of Santa Clara Valley (IHCSCV). This recognition is a testament to our dedicated efforts in improving the rates of HEDIS (Healthcare Effectiveness Data and Information Set) measures, which assess the quality of services we provide to our patients.
This achievement aligns with the broader framework of health centers working on HEDIS measures. Another article highlights the crucial work of our patient navigators in managing hypertension and diabetes, integral components of our strategy to improve patient outcomes. We are immensely proud of our team's hard work and dedication. This recognition from SCFHP motivates us to continue striving for excellence in all aspects of patient care.
