Webinar: The Accountable Health Communities Screening Tool

Join us to learn about the new social needs screening tool developed by Centers for Medicare and Medicaid Services (CMS) for use in the Accountable Health Communities (AHC) model program. Alexander Billioux and Katherine Verlander of the Center for Medicare and Medicaid Innovation at CMS will present on the development and implementation of this recently released 10-item screening tool designed to identify patient needs in the following five domains: housing, food, transportation, utilities, and interpersonal safety. Topics covered in the webinar will include how the tool was developed, implementation considerations, and ways the tool will be used in the AHC demonstration.
